Idaho is embarking on its State Healthcare Innovation Plan – the SHIP – a redesign of Idaho’s healthcare system, evolving away from fee-for-service, volume based medicine to a value based system of care rewarding improved health outcomes.
The first SHIP goal is to transform practices across the State into patient centered medical homes (PCMH). Other goals, such as care coordination through EHR, building regional infrastructure to support the integration of PCMH with the medical neighborhood, development of virtual PCMH, and alignment of payment mechanisms, support the first goal.
To support Idaho’s healthcare transformation, IPCA is bringing NCQA PCMH training to Idaho, and with reduced registration costs!

Introduction to PCMH: Foundational Concepts of the Medical Home
PCMH and Idaho: How PCMH relates to SHIP and the Idaho environment
June 9-10, 2015
Following training, the participant will be able to:
· Examine the essential characteristics of a recognized PCMH.
· Identify the measurement and documentation criteria for each of NCQA’s updated requirements.
· Facilitate the NCQA survey and evaluation process.

Advanced PCMH: Mastering the Medical Home Transformation*

*Introduction to PCMH is a prerequisite.
Following training, the participant will be able to:
· Identify and address the PCMH concepts, challenges, and standards.
· Establish and maintain a CQI culture within a PCMH practice.
· Interpret meaningful use criteria and EHR pre-validation to obtain credit in the aligned elements.
· Describe all phases of the submission and review process, and post-notification options and actions.
· Identify opportunities for alignment with other recognition programs.
Cost: $550 / participant
